To apply for the weather lottery simply print out a copy of the form shown below, complete all three sections and send it to and send it to: Lottery Service Providers, Freepost NEA 12220, LEEDS, LS6 6YY.
To pay by cheque, please fill in the form with your name and address (including postcode) and your chosen numbers. Send the form, together with a cheque payable to "Prize Provision Services Ltd" to the address above. You can choose to pay £13 for 13 weeks per line, £26 for 26 weeks per line or £52 for 52 weeks per line. You will be automatically sent a reminder 4 weeks before your last play date.
